Kisspeptin Hormone Injection Could Treat Low Sex Drive in Women and Men

The hormone kisspeptin could be used to treat women and men distressed by their low sexual desire, according to two new studies.

The studies, led by clinicians and scientists at Imperial College London and Imperial College Healthcare NHS Trust and both published in JAMA Network Open, found that giving kisspeptin can boost sexual responses in women and men who have hypoactive sexual desire disorder (HSDD)—a condition characterized by low sexual desire that is distressing to the individual. HSDD affects up 10% of women and 8% of men worldwide and can have devastating psychological and social impacts.

Kisspeptin is a naturally-occurring hormone that stimulates the release of other reproductive hormones inside the body. The team have previously shown in men with intact sexual desire that kisspeptin can enhance responses to sexual stimuli, and boost attraction brain pathways, independent of other reproductive hormones like testosterone. Now, they investigated the effects in women and men with low sexual desire for the first time.

These two clinical trials involved 32 pre-menopausal women and 32 men with HSDD. In both studies, patients underwent scanning of the brain using MRI, as well as blood and behavioral tests. Kisspeptin administration improved sexual brain processing in both women and men, resulting in positive effects on sexual behavior compared to placebo. These are the first clinical studies to explore the ability of kisspeptin to boost sexual pathways in women and men distressed by their low sexual desire.

The researchers believe that the results lay the foundations for kisspeptin-based treatments for women and men with HSDD.

Dr. Alexander Comninos, from the Department of Metabolism, Digestion and Reproduction at Imperial College London, Consultant Endocrinologist at Imperial College Healthcare NHS Trust and co-senior author of the study, said, “Low sexual desire can be distressing and so result in HSDD. This can have a major detrimental impact on relationships, mental health, and fertility.

“Even though it is relatively common, treatment options in women are limited, carry significant side-effects and in some cases can be harmful to even try. And unfortunately, these treatments have limited effectiveness. In men there are currently no licensed treatments and none on the horizon.

“Therefore, there is a real unmet need to find new, safer and more effective therapies for this distressing condition for both women and men seeking treatment.

“Our two studies provide proof-of-concept for the development of kisspeptin treatments, as we provide the first evidence that kisspeptin is a potentially safe and effective therapy for both women and men with distressing low sexual desire. Additionally in men, we demonstrate that kisspeptin can have positive effects not only in the brain but also in the penis by increasing rigidity.

Furthermore, kisspeptin was well-tolerated by both women and men with no side-effects reported, which is crucial from a drug development point of view. We now plan to take things forward to hopefully realize the potential of kisspeptin therapeutics in psychosexual disorders—sexual problems which are psychological in origin, such as unexplained low libido.”

Professor Waljit Dhillo, an NIHR Senior Investigator, also from the Department of Metabolism, Digestion and Reproduction at Imperial College London, Consultant Endocrinologist at Imperial College Healthcare NHS Trust and co-senior author of the study, added, “Our studies build on our previous work to assess the effectiveness of kisspeptin and its boosting effects in terms of arousal and attraction. It is highly encouraging to see the same boosting effect in both women and men, although the precise brain pathways were slightly different as might be expected.

“Collectively, the results suggest that kisspeptin may offer a safe and much-needed treatment for HSDD that affects millions of people around the world and we look forward to taking this forward in future larger studies and in other patient groups.”

The study in women involved a randomized, double-blind, two-way crossover, placebo-controlled trial at Invicro and Hammersmith Hospital (part of Imperial College Healthcare NHS Trust), between October 2020 and April 2021. Data analysis was carried out by Imperial College London researchers.

Thirty-two pre-menopausal heterosexual women with HSDD (aged 19–48 years) completed two study visits, one for administration of kisspeptin and another visit for placebo.

Participants completed psychometric questionnaires before and towards the end of kisspeptin or placebo administration to assess their mood and behavior. During kisspeptin or placebo administration, participants underwent functional MRI while watching erotic videos and viewing male faces to see how brain activity was affected. Non-erotic exercise videos were used as a control.

The team found that kisspeptin improved sexual and attraction brain activity in key brain areas in women. They also found that women who were more distressed by their sexual function showed greater kisspeptin-enhanced brain activity in the hippocampus (a key structure implicated in female sexual desire).

Furthermore, the more kisspeptin activated the posterior cingulate cortex—a key behavioral brain area—in response to attractive male faces, the less sexual aversion was reported by participants.

Crucially, the psychometric analyses revealed that the women reported feeling “more sexy” during kisspeptin, compared to placebo.

In the second study, which was also a randomized, double-blind, two-way crossover trial, 32 heterosexual men with HSDD (aged 21–52 years) underwent a similar study with the addition of the measurement of penile rigidity, between January and September 2021.

The study demonstrated that kisspeptin significantly boosted brain activity in key structures of the sexual brain network while also increasing penile rigidity by up to 56% compared to placebo, while viewing an erotic video.

Similarly to the study in women, kisspeptin also had greater effects in key brain regions in men more distressed with their low sexual desire. Furthermore, psychometric analyses revealed that kisspeptin improved “happiness about sex” reported by the men.

Dr. Comninos and Professor Dhillo now plan to take this forward with larger scale studies, studies in different populations and collaborations to develop kisspeptin as a realistic treatment for both women and men with distressing psychosexual disorders.

Effects of Kisspeptin on Sexual Brain Processing and Penile Tumescencein Men With Hypoactive Sexual Desire Disorder

Importance  

The human physiological sexual response is crucial for reward, satisfaction, and reproduction. Disruption of the associated neurophysiological pathways predisposes to low sexual desire; the most prevalent psychological form is hypoactive sexual desire disorder (HSDD), which affects 8% of men but currently has no effective pharmacological treatment options. The reproductive neuropeptide kisspeptin offers a putative therapeutic target, owing to emerging understanding of its role in reproductive behavior.

Objective  

To determine the physiological, behavioral, neural, and hormonal effects of kisspeptin administration in men with HSDD.

Design, Setting, and Participants  

This double-blind, 2-way crossover, placebo-controlled randomized clinical trial was performed at a single academic research center in the UK. Eligible participants were right-handed heterosexual men with HSDD. Physiological, behavioral, functional magnetic resonance imaging (fMRI), and hormonal analyses were used to investigate the clinical and mechanistic effects of kisspeptin administration in response to visual sexual stimuli (short and long video tasks). The trial was conducted between January 11 and September 15, 2021, and data analysis was performed between October and November 2021.

Interventions  

Participants attended 2 study visits at least 7 days apart, in balanced random order, for intravenous infusion of kisspeptin-54 (1 nmol/kg/h) for 75 minutes or for administration of a rate-matched placebo.

Main Outcomes and Measures  

Changes in (1) brain activity on whole-brain analysis, as determined by fMRI blood oxygen level–dependent activity in response to visual sexual stimuli during kisspeptin administration compared with placebo, (2) physiological sexual arousal (penile tumescence), and (3) behavioral measures of sexual desire and arousal.

Results  

Of the 37 men randomized, 32 completed the trial. Participants had a mean (SD) age of 37.9 (8.6) years and a mean (SD) body mass index of 24.9 (5.4). On viewing sexual videos, kisspeptin significantly modulated brain activity in key structures of the sexual-processing network on whole-brain analysis compared with placebo (mean absolute change [Cohen d] = 0.81 [95% CI, 0.41-1.21]; P =  .003). Furthermore, improvements in several secondary analyses were observed, including significant increases in penile tumescence in response to sexual stimuli (by up to 56% more than placebo; mean difference = 0.28 units [95% CI, 0.04-0.52 units]; P = .02) and behavioral measures of sexual desire—most notably, increased happiness about sex (mean difference = 0.63 points [95% CI, 0.10-1.15 points]; P = .02).

Conclusions and Relevance  

Collectively, this randomized clinical trial provides the first evidence to date showing that kisspeptin administration substantially modulates sexual brain processing in men with HSDD, with associated increases in penile tumescence and behavioral measures of sexual desire and arousal. These data suggest that kisspeptin has potential as the first pharmacological treatment for men with low sexual desire.

By using the online psychotherapy platform “Cure of Mind,” the client acknowledges and agrees to maintain strict confidentiality and privacy regarding all information shared during the therapy sessions. This includes any personal, medical, or sensitive data disclosed to the therapist (psychologist or medical doctor or nutritionist) during the course of treatment.

“Cure of Mind” commits to safeguarding the client’s personal information and adheres to all applicable laws and regulations pertaining to data protection and privacy. The client’s data will be securely stored and accessible only to authorized personnel involved in the therapy process.

The client agrees not to record, share, or distribute any part of the therapy sessions, including audio, video, or written content, without explicit written consent from both “Cure of Mind” and the therapist (psychologist or medical doctor or nutritionist).

In the event of any legal obligation to disclose confidential information, “Cure of Mind” will make reasonable efforts to notify the client beforehand, except in cases where immediate disclosure is required by law or poses a risk to the client’s safety or the safety of others.

“Cure of Mind” encourages open and honest communication during therapy sessions; however, the client is responsible for ensuring their online environment is private and free from potential eavesdropping or unauthorized access during sessions.

The client understands that while “Cure of Mind” takes all necessary precautions to maintain confidentiality, the use of online platforms carries inherent risks related to data transmission, and “Cure of Mind” cannot guarantee absolute security. Therefore, the client assumes responsibility for any risks associated with sharing information through the online platform.

By proceeding with therapy sessions on “Cure of Mind,” the client acknowledges and accepts the terms of this confidentiality and privacy agreement, providing their informed consent to engage in therapy under these conditions.

Professional Liability

“Cure of Mind” is a platform that connects clients with selected licensed psychologists, medical doctors, and nutritionists who provide professional psychotherapy, nutritional and medical services. The liability for the provision of these services lies solely with the individual professionals conducting the therapy sessions.

All therapists, medical doctors, and nutritionists associated with “Cure of Mind” are independent practitioners, responsible for their own actions and conduct during therapy sessions. “Cure of Mind” does not directly oversee or monitor the content or conduct of individual therapy sessions due to privacy reasons. As such, “Cure of Mind” cannot be held liable for any professional misconduct, malpractice, or ethical violations that may occur during therapy sessions.

Clients engaging in therapy sessions through “Cure of Mind” understand that the psychologists, medical doctors, and nutritionists are bound by their respective professional codes of ethics, licensing regulations, and applicable laws governing their practice. Clients are encouraged to discuss any concerns or issues with their respective psychologist, medical doctor, or nutritionist directly.

“Cure of Mind” may provide general guidelines and policies to promote a safe and positive therapeutic environment, but clients acknowledge that the final responsibility for adherence to ethical and professional standards lies with the individual professionals providing the services.

By using the “Cure of Mind” platform and engaging in sessions with the licensed psychologists, medical doctors, and nutritionists, clients implicitly acknowledge and accept that “Cure of Mind” cannot be held responsible for any professional liability or misconduct that may arise during the course of therapy sessions. Clients agree to release “Cure of Mind” from any claims or damages related to the professional conduct of the therapists, medical doctors, and nutritionists providing services on the platform.

Disclaimer of warranty

To the maximum extent permitted by applicable law, the aggregate liability of Cure of Mind LP and its affiliates, officers, employees, agents, suppliers and licensors relating to the services will be limited to an amount no greater than one dollar or any amounts actually paid in cash by you to Cure of Mind LP for the prior one month period prior to the first event or occurrence giving rise to such liability. The limitations and exclusions also apply if this remedy does not fully compensate you for any losses or fails of its essential purpose.

Indemnification

You agree to indemnify and hold Cure of Mind LP and its affiliates, directors, officers, employees, agents, suppliers and licensors harmless from and against any liabilities, losses, damages or costs, including reasonable attorneys’ fees, incurred in connection with or arising from any third party allegations, claims, actions, disputes, or demands asserted against any of them as a result of or relating to your Content, your use of the Services or any willful misconduct on your part.

Severability

All rights and restrictions contained in this Agreement may be exercised and shall be applicable and binding only to the extent that they do not violate any applicable laws and are intended to be limited to the extent necessary so that they will not render this Agreement illegal, invalid or unenforceable. If any provision or portion of any provision of this Agreement shall be held to be illegal, invalid or unenforceable by a court of competent jurisdiction, it is the intention of the parties that the remaining provisions or portions thereof shall constitute their agreement with respect to the subject matter hereof, and all such remaining provisions or portions thereof shall remain in full force and effect.

Dispute resolution

The formation, interpretation, and performance of this Agreement and any disputes arising out of it shall be governed by the substantive and procedural laws of Greece without regard to its rules on conflicts or choice of law and, to the extent applicable, the laws of Greece. The exclusive jurisdiction and venue for actions related to the subject matter hereof shall be the courts located in Greece, and you hereby submit to the personal jurisdiction of such courts. You hereby waive any right to a jury trial in any proceeding arising out of or related to this Agreement. The United Nations Convention on Contracts for the International Sale of Goods does not apply to this Agreement.

Assignment

You may not assign, resell, sub-license or otherwise transfer or delegate any of your rights or obligations hereunder, in whole or in part, without our prior written consent, which consent shall be at our own sole discretion and without obligation; any such assignment or transfer shall be null and void. We are free to assign any of its rights or obligations hereunder, in whole or in part, to any third party as part of the sale of all or substantially all of its assets or stock or as part of a merger.

Changes and amendments

We reserve the right to modify this Agreement or its terms related to the Services at any time at our discretion. When we do, we will revise the updated date at the bottom of this page, post a notification within the Services. We may also provide notice to you in other ways at our discretion, such as through the contact information you have provided.

An updated version of this Agreement will be effective immediately upon the posting of the revised Agreement unless otherwise specified. Your continued use of the Services after the effective date of the revised Agreement (or such other act specified at that time) will constitute your consent to those changes.

Your therapist is a licensed mental health professional who has completed an advanced degree in psychology or a related field and has been trained to provide psychotherapy. Your nutritionist and medical doctor are also licensed professionals with expertise in their respective fields. They are committed to providing you with the highest quality of care possible.

Goals and Objectives:

The goals of psychotherapy are to help you overcome emotional or psychological difficulties that are causing distress in your life. Nutritional counseling aims to support your dietary needs, promote healthy eating habits, and address specific nutritional concerns. Medical counseling focuses on providing you with medical guidance, treatment options, and managing your overall health.

Treatment Methods:

Your therapist will use a variety of treatment methods in psychotherapy, including talk therapy, cognitive-behavioral therapy, psychodynamic therapy, mindfulness techniques, or other approaches. Your nutritionist will provide nutritional education, create personalized meal plans, and offer guidance on making healthy food choices. Your medical doctor will assess your medical history, perform necessary examinations, refer you for lab tests and recommend appropriate medical treatments.

Risks and Benefits:

Psychotherapy, nutritional counseling, and medical counseling can be beneficial in various ways, such as improving your emotional well-being, enhancing your nutrition, and promoting better physical health. However, there are also some risks associated with these treatments. Risks may include experiencing uncomfortable emotions during therapy, potential changes in your relationships, and the possibility of addressing challenging nutritional or medical issues.

Confidentiality:

Your therapist, nutritionist, and medical doctor are committed to protecting your privacy and will keep all information discussed during your sessions confidential, with some exceptions. These exceptions include situations where there is an imminent risk of harm to yourself or others, or where there is a legal obligation to disclose information.

Duration and Frequency of Sessions:

The length and frequency of therapy sessions, nutritional counseling sessions, and medical counseling appointments will depend on your individual needs and goals. Typically, therapy and counseling sessions are scheduled weekly or biweekly and last for approximately 50 minutes. While nutritional counselling appointments last up 45 minutes and medical counseling appointments last up to 30 minutes and their frequency may vary.

Fees and Payment:

Fees are charged on a session-by-session or appointment basis and may be covered by insurance if this is available to you.

For insurance coverage, it is important you understand your insurance policy and coverage. You should forward your detailed invoice from Cure of Mind to your insurance provider and discuss with them whether the services provided by the therapist, nutritionist, and medical doctor are eligible for coverage as per the terms of your insurance plan.

By ticking the box “Informed Consent” on the booking form, you acknowledge that you have read and understand the information provided and that you have had the opportunity to read the information about psychotherapy, nutritional counseling, and medical counseling provided on Cure of Mind and to ask questions in our Help Center, or during your first appointment with one of our professionals. You consent to participate in these treatments and understand that you may withdraw your consent at any time.
